*My assistants Ellen or Katie are in the office Monday 1:30-4:30pm and  during my above office hours, but in order for them to finish work on-time, the phones are monitored until 4:30pm.

My office is wheelchair accessible, with a Lift (it's like an elevator), that requires a key.  Please discuss the logistics ahead of time with me if you will need access to the Lift.

(Please note, that I will NOT fax patient records with only a faxed release form, unless it is a psychiatric emergency and the records are going directly to an inpatient psychiatric unit.  Any other releases faxed to me must be followed by a verbal request to me from either another Prescriber, or therapist, or by the patient themselves.  I will confirm with the patient that they in fact have signed the release for me to share information about them with you.)

My fax number is provided primarily  as a way referral sources and providers can get your confidential information to  me.  For patients, please call me or leave me a voicemail message with any questions or requests.

My fax system is secure and can, at any time, receive clinical information and referral requests that includes your personal information.

Due to confidentiality laws and for other clinical reasons, I am not able to receive emails that include any clinical information about you.

**When you contact me or my assistant, please clearly and slowly state your name, a phone number (and please repeat the phone number to make sure we hear it correctly) and let us know a few times when you can be reached.  Please let us know if we cannot leave voicemail at the number you provide.

New patient appointments are on Wednesday afternoons.

By Car (Free Parking):

From the  North

1.)  Take Interstate 91 south to exit 20. Stay in the right lane as you get off of the Interstate.

2.)  At the first light take a right onto Bridge Road.

3.)  At the next set of lights, take a left turn onto Jackson Street.

4.)  At the stop sign, turn right onto Prospect Street, which quickly merges onto Locust Street (Route 9 towards Florence)

5.)  My office is 51 Locust Street, the one-story green building, with white columns, across the street from Cooley Dickinson Hospital.  If you pass Smith Vocational School or The Recycling Center, you have gone too far.

6.)  Turn right into the parking lot, just after our one story green building with white columns.

From the South

1.) Take Interstate 91 north to exit 19, Amherst/Northampton.

2.) At the end of the ramp, go forward.

3.) You are now on Damon Road. At the end of Damon Road, you will come to a second set of lights. Go straight through these lights; the road becomes Bridge Road.

4.) At the next set of lights, take a left turn onto Jackson Street and follow Jackson Street until you come to a stop sign.

5.) At the stop sign take a right onto Prospect Street, which quickly merges onto Locust Street (Route 9 towards Florence)
