*Outpatient detoxification
*Disability examinations for non-established patients (under my care already for at least 6 months)
*Fitness-for-duty evaluations
*Forensic Evaluations
*Suboxone
*Xanax/Alprazolam
*Vivitrol
*Clozaril
*Injectable forms of medications
*Medications to treat dementia
* Treatment for Anorexia Nervosa
* I do not have expertise in treating patients with Autism Spectrum Disorders.
Payment:
I accept all credit cards. Payment is due one the day of each appointment with me. My office will keep an encrypted token version of your credit card, to have on-file, which will make it easier for you and for my office staff to manage payments. If you need statements (full-receipts), my office staff will send your encrypted statements to your "Patient Portal" for your privacy and convenience.
Insurance:
I do not "accept" any insurances because of their low rates of reimbursements. But, if you have or can get "out-of-network" insurance coverage, I can provide you with a statement (full-receipt) after each appointment that you can submit for partial reimbursement.
Will my insurance cover my visits with Dr. Smith?
If you have an HMO type of insurance, then the answer is "no".
Only the following types of insurances will reimburse you a portion of my fees, but you will probably first have to pay an annual deductible every year: 1.) Preferred Provider Organizations (P.P.O.), 2.) Point of Service Plans (P.O.S.). These two types of plans allow patients to see "out-of-network" providers.
If you are not sure of your benefits, please contact your insurance and ask them if they can reimburse you if you see me.
I will provide you with a "Full receipt" at the end of our visits, which includes the information that P.P.O. and P.O.S. insurance plans request from out-of-network providers. Please ask your insurance company what information it will need for you to qualify for reimbursement, or it may delay their reimbursements to you. If your insurance has approved reimbursements for you to see me, you then submit each of my receipts with a one page claim form (from your insurance company that you fill out) to your insurance; and they will send you a reimbursement check several weeks later, for a portion of my fees. My patients have informed me they often receive between 60% to 80% of my fees back to them, depending on the type of insurance.
It is your responsibility to understand what your insurance requires for you to qualify reimbursement from them, including referrals, special forms that need to be completed, requests for additional care, deductibles, and any other requirements.
If you do not have "out-of-network coverage" you can still see me and pay "out-of-pocket", but you will not be reimbursed by your insurance company.
Please note, that our office may charge you for missed appointments, but most insurances will not reimburse you for a missed appointment fee.
